# Root Cause Analysis

For each incident, the RCA should include the following information:

* **Type of Issue:** The nature of the incident (e.g., Access Control, Reentrancy, Price Manipulation, Flashloans).
* **Date of Issue:** When the incident occurred.
* **Title of Issue:** A concise name for the incident.
* **Root Cause:** Explanation of the underlying cause(s). Identify the fundamental issues.
* **Amount Lost:** The financial impact of the incident.
