# Questionaire

1. On what devices do you have your affected metamask account imported?
2. On what device was the account created?
3. How did you import the account to the other devices? For example, did you send your seed to yourself using an app, did you type it in from a piece of paper, something else?
4. Was the metamask seed at any point saved on a digital medium besides the metamask app/extension (for example a notes app, temporarily sent via chat and then deleted, password manager, etc)?
5. Please list all the affected ethereum addresses.
6. Provide the timestamps (and/or hashes) of all transactions that where sent by the attacker.
