Governing Body Charter Template

Anyone wishing to form a governing body can use this template to submit a proposal to be chartered, funded if requested, and to begin operations.

A template has been provided in google docs format that meets all of the requirements of the [Governing Bodies Policies Section 1.3](https://hackmd.io/@PA-vPATreasury/HyArqDA7s#1%E2%80%8B-Chartering).
