Note Table of Contents 20240505 Prognososis adalah dokumen pada step akhir perawatan cancer pada RS, namun perlu monitoring lebih lanjut kesehatan pasien cancer seusai masa pengobatan di rs, melalui telemedicine atau PHR aplikasi, serta perlu pendefinisisan workflow dan spek data yang perlu di monitoring bagi pasien cancer pasca treatment secara jarak jauh. Penggunaan FHIR pada record obat sama dan sejalan antara yang diimplementasi di Taiwan dan di Indonesia. Hanya saja Taiwan perlu real usecase penggunaan FHIR dalam merecord data obat, seperti contoh perecordan obat pada usecase pengobatan pasien cancer. Di Taiwan mekanisme sekuriti pembuatan dan pengeditan data order obat dilakukan dengan pencolokan kartu sertifikat dokter (physical card). Namun, cara ini tergolong menghambat dan kurang efisien bila dilakukan di lapangan. Alangkah lebih baiknya bila mekanisme sekurity dilakukan dengan pendefinisian scope yang jelas bagi user dalam mengkases data order obat (FHIR). Mekanisme penggunaan physical card ini dilakukan di Taiwan dikarenakan, saat itu belum ada FHIR dan teknology belum terlalu modern. Di Taiwan juga dokter perlu sign (pencolokan kartu / atau login dengan e-sertivikat pada HIS mobile app) untuk dapat membuat diagnosis. Saat ini, Taiwan sedang mengembangkan workflow dan spek order dan ambil obat pada usecase chronic continue medication

A1: For disease tracking record, because curently the condition, observation, diagnosis and treatment plan (S.O.A.P) are recorded in written form and are less effective for describing skin condition. Several photos were used for biopsy records (before and after). Photos are also used to publish papers and research (photos without the patient's face) Q2: Is the image used in presentations or consultations between doctors? A2: Yes, used for reports on regarding the development of a particular patient disease in internal department meeting （主治醫師）. Curently neither the report nor the captured image is stored in the medical record. O1: Internal reports may not need to be linked to the medical record, but captured images during the encounter should be recorded in the medical record. Q3: When do general practitioners usually capture a patient's skin condition? for each patient, or patients with certain conditions? A3: Surgery patients, tumors, special skin conditions

Integration of Document Repository & Document Registry on Healthcare Ecosystem Introduction Setiap rumah sakit memiliki tempat penyimpanan dokumen masing-masing, seperti server VNA, File server, dll. Dokumen yang disimpan dapat berupa ringkasan medis, report laboratory, report radiology, photo medis, dll. Untuk menunjang pelayanan medis dan kesehatan pertukaran dokumen antara rumah sakit sangat diperlukan. Maka dari itu pada artikel kali ini, akan menjabarkan mekanisme pertukaran dokumen antar repositori dengan mengimplementasi dokument registry. Sebelum itu mari kita pahami definisi masing-masing. Dokumen repository = storage tempat menyimpan dokumen, seperti File server, Google drive, VNA, etc Dokumen repository digunakan bagi menyedia jasa medis untuk menyimpan dokumen, baik dokumen pasien maupun dokumen administrasi rumah sakit. Document registry = storage tempat mengindex dokumen, seperti excel, database, etc Dokumen registry digunakan untuk memudahkan managemen, pencarian dan pengaksesan dokumen. Dengan dokumen registri dokument yang tersebar dan tersimpan pada several storage pun dapat mudah diakses, tersentralize, dan akses terpusat.

Background: Figure 1. Image annotation `Situation：` > Doctors sometimes diagnose and treat diseases based on the patient's medical imaging. The Radiologist is the doctor who supervises these exams, reads, interprets the images, and writes a report to the main doctor. When a radiologist wants to make a report of a medical image (CT, MRI, X-Ray, etc). Doctor need to display medical imaging, then annotate the abnormality occurence on the image, once there is an annotation, the doctor can write a image finding report based on the annotation.
