During your visit and in the context of the several foressen activities, we will be collecting personal data, namely in the form of text, photos and videos. Please spend a little of your time going through this consent form.

## Purposes of Processing and the Storage of Data

All data collected will be used for the sole **purposes** of efficiently organizing and disseminating the event and UNITA activities.
The data collected in the context of the aforementioned activities will be stored in a local database, accessed and processed only by the staff of the Public Relations Office and the staff of the UNITA's office at University of Beira Interior (UBI), for as long as the project is in effect, transitioning to a secure archive after that. No processes aimed at profiling based on this data will be used.

## Identification of the Data Controller

The data controller is the University of Beira Interior (UBI), a public higher education institution located in the city of Covilhã, Portugal.

## Categories and Means of Collecting of Personal Data

Personal data collected and processed in the scope of the aforementioned activities include names, information related with pseudo-localization (attendance lists), email addresses, photos and videos in which interested parties are identifiable.
While the attendance in several activities will be collected in paper, names and email addresses might originate from forms or structured data sources available in the context of the project (e.g., data exchanged during the preparation of the event). Photos and videos will be collected by technical staff in charge of that task, which are easely identifiable during the event.

Some meetings will use videoconferencing tecnologie to enable the participation of remote member. However, it is not forseen to record these meetings.

## Entities to which the Data can be Communicated

Portions of the collected data will be used to prepare articles for the dissemination of the event in social networks or the press. No data will be sent to any other entity in any other form.

## Exercising the Right of Access, Rectification or Forgetting

The data subject can freely exercise their rights of access, rectification or forgetting by sending a request to the staff of the UNITA's office at UBI, with carbon copy to the [e-mail].

By signing this document I declare:
- to have been informed and made awere of the use of the personnal data;
- that I have autorized the filming of my own image, name and voice, for the institutional purposes related to the event and
- to have read and acceped the terms and conditions for the prossessing of personnal data in the manner and for the purposes indicated above.
